Examples of healthcare fraud, abuse and waste include:
Using someone else’s medical insurance information to obtain services or medication;
Billing for a service that was not provided;
Over-billing for a service;
Providing a service that was not medically necessary;
Falsifying signatures or medical records;
Unethical personal, romantic, sexual or business relationships between ChangePoint staff and members.
If you are aware of or even suspect that fraud, abuse or waste involving a ChangePoint employee or member has occurred, we want you to notify us. We will review your concerns and take appropriate action.
